Whenever possible, request a Record of Participation on the letterhead of the facilitator of the program.  If they cannot provide that for you right away - use what we have supplied temporarily.

Use the FRC Record of Participation Form as a method of note taking and quick reference for later record keeping.

Record Credit Hours earned for instruction and participation time.

Be sure to have a copy of the form with you so the program facilitator can sign and date.

Don't forget your camera!

Grab a brochure, map, calendar of events or program description at the program site.

On the way home from a program, ask your child's opinion of the program and make note of those comments on the FRC Record of Program Participation Form.

Express your own observations about the program.  Include any observations about your child and the program's presenter and content.

The form allows space for you to jot down additional educational materials used in preparation for this program.  Include related project work as well.
